Primary Responsibilities:Clinical Care - Responsible for the overall coordination and delivery of patient services delegating responsibility as appropriate to program coordinators.- Works in conjunction with the Director of Patient and Family Services to provide clinical leadership for the organization.- Oversees delivery of patient services and compliance with agency policy in all patient care settings, including care provided in the patients† homes, in various hospitals and nursing homes, and in Hospice Hawaii†s Homes.- Assures compliance with all Hospice Hawai†i policies and procedures related to clinical services.- Assures scheduling, supervision and ongoing education of departmental staff. - Supervises the Medical Records Department, overseeing the maintenance of medical records, assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.- Plans for recruitment and career development of patient care team personnel.- Chairs regular meetings of the Clinical Department.- Chairs regular meetings of the Clinical Team whose responsibility is the establishment of procedures governing the day-to-day provision of hospice care and services.- Provides guidance in communication, coordination, and effective operations related to patient care through development and utilization of recording and reporting systems, manuals and other resource materials.- Develops policies, procedures and guidelines to fulfill program objectives.- Serves as backup on call 24 hours/day 7 days/week as needed.- Participates in the quality assurance program, revising operations as indicated.Professional Relationships- Serves as liaison with community nurses, medical schools, and teaching facilities.- Actively develops and maintains long-term, beneficial collaborative peer relationships throughout the island of Oahu and other areas served.Program Development- Actively engages in the development of programs within Hospice Hawaii in order to accomplish strategic objectives or other desired goals of the organization.- Participates in the quality improvement/ utilization review program.Clinical Education- Contributes to the development of in-service education for Hospice Hawaii staff and volunteers.- Actively engages in public and professional education, directly through Hospice Hawaii's efforts and/ or in collaboration with others in the community.- Supports Medical Students and Interns as opportunity presents in collaboration with local universities.- Identifies educational needs of nursing facility staff related to hospice care and Hospice Hawai†i philosophy and assists in training nursing facility staff.Administration- Develops the annual budget proposal for the Clinical Department. Is responsible to maintain the departmental expenditures within budget or allowed variances.- Assists in the development of personnel policies and monitors compliance in areas of responsibility. Participates in developing and revising written patient care objectives and procedures.- Serves as a member of the Hospice Hawaii Leadership Team.- Responsible for supporting compliance with licensing and Medicare certification, including all rules and regulations from Chapter 94 and CMS, as applicable.Leadership Responsibilities- Provides leadership, problem solving, and management decision-making and demonstrates a commitment to quality and corporate accountability.- Actively participates in staff meetings, department meetings, in-service training, and other related activities.- Assumes management responsibilities for other areas of Hospice Hawai†i as assigned.- Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security, risk management and personnel policies.- Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.- Expected to participate in the Performance Improvement Program and to comply with Federal and State regulations governing hospice programs, and to adhere to the organization†s standards of conduct as a condition of continued employment.- Demonstrates good judgment in seeking and accepting guidance, facilitates clear and honest communication with other team members.

Demonstrates ethical conduct and judgment that is in harmony with the mission and values of Hospice Hawaii.- Demonstrates ability to execute an effective, supportive, sensitive approach in response to the needs of staff and the patient and family. - Works harmoniously with a broad range of staff, volunteers, and the community as a representative of Hospice Hawaii. - In concert with the President or designee, overseas the functioning of the agency†s Hospice Facilities.- Participates in external activities and committees to support the agency†s community service mission. Is available to participate in educational outreach programs.- Performs all other duties as assigned by the President.Management- Assumes responsibility for agency administration in the absence of the President as assigned.- Identify need for changes in program and service, develop proposals for consideration by Management.- Plan and implement clinical and administrative projects, coordinating them with overall agency plans.- Serve as a member of the Management Team and participate in overall agency planning.- Participate in other projects and/or provide information for projects as outlined by the President or Management Team.- Provides leadership, problem solving, and management decision-making and demonstrates a commitment to quality and corporate accountability.- Participates in the orientation program for new hospice staff personnel.- Consistently represents Hospice Hawaii to all internal and external customers with integrity and professionalism- Performs other duties as required.
