• Licensed/Pending - RN - State of Hawaii
• Basic computer knowledge/skill in Microsoft Windows

POSITION SUMMARY: Provides liaison and leadership role! for the clinical team. Provides for clinical continuity of care and coordination among disciplines, hospital and other referral sources. Coordinates home care admissions, completes plan of care (SOC OASIS), team utilization (visits), quality outcomes, manages cases within fiscal resources. Actively seeks applications for managing cases to improve outcomes and patient satisfaction. Manages team of field staff. Collaborates with other case managers.
MINIMUM EDUCATION: Bachelor's degree from an accredited program including community health/public health nursing, PT, OT, ST.
MINIMUM FIELD-OF-EXPERTISE: Knowledge and skill in the use of Microsoft Windows applications. Knowledge of computers and their applications to clinical systems. Ability to communicate effectively in verbal and written English with patients/families/care team of varying cultures, medical needs and expectations. Understands the application of case management in the home care environment! . Proven leadership experience.
REQUIRED CERTIFICATION/! REGISTRATION: Currently licensed by the State of Hawaii as a Registered Nurse or Clinician. Current CPR, valid Driver's License and automobile no-fault insurance.
PREFERRED EXPERIENCE: At least five (5) years of experience as a registered nurse in a community health service, home heath agency or equivalent leadership experience in the past five years. Varied nursing background in such areas as medical/surgical, psychiatric and intensive care nursing preferred. Demonstrated leadership skills and knowledge of community resources, case management, and regulatory issues.
