Job Description :

BEHAVIORAL HEALTH UM/CM NURSE *RELOCATION NOT PROVIDED

Responsible to the Manager of Utilization Management (UM) for general pre-certification, concurrent and/or retrospective review of inpatient and/or behavioral health (BH) outpatient services.

The BH UM/CM Nurse works closely with the Medical Director to evaluate inpatient treatment and clinical progress of members admitted to acute and sub-acute care facilities. Works closely with the departmental management to impact the treatment plan and identify treatment plan alternatives.

Will also work with the Care Coordination team on Severely Persistent Mental Illness (SPMI) members referred to Case Management or Disease Management who need more intensive Complex Case Management. Work as a complex case manager and manages individuals with complex BH conditions. Collaborate with inpatient team, at tends care meetings and discharge planning meetings. Communicate with home health team, PCP and appropriate specialists about the member’s goals and plan.


TYPICAL WORKING CONDITIONS: Office based position; subject to inside environmental conditions (temperature fluctuations of air-conditioned office), not substantially subjected to adverse external environmental conditions. Subject to noise from various types of office equipment, including but not limited to: typewriters, copy machines, faxes, printers and computers. May be exposed to chemicals and/or fumes (e.g. copy toner or cleaning fluid). May require travel via own automobile or day/overnight travel to the Neighbor Islands and/or the Mainland.


TYPICAL MENTAL, PHYSICAL AND COMMUNICATION DEMANDS:

  • Ability to multitask and function independent and responsibly with minimal supervision in a high volume/busy environment. Ability to work calmly under pressure and appropriately plan, prioritize, organize and carry out tasks and responsibilities. Ability to be objective and professional. Ability to organize work and develop strategies for adapting to a constantly changing workload or when confronted with unforeseen situations.
  • Essential: Sitting, finger dexterity, hearing, speaking, static gripping of an object for prolonged periods.
  • Continuous: Finger dexterity, hearing.

  • Occasional: Stooping/bending, kneeling, crawling, climbing stairs, crouching/squatting, twisting body, lifting usual amount of 5 lbs., pushing/pulling usual amount of 5 lbs., carrying usual amount of 5 lbs., reaching above shoulder, reaching at shoulder level, static gripping of an object for prolonged periods, frequent gripping of an object.
  • Good decision making and communicating skills. Ability to motivate people. Ability to read and interpret documents such as safety rules, operating an d maintenance instructions and procedure manuals. Ability to write routine reports, correspondence and procedure manuals. Ability to communicate effectively both verbally and in writing. Ability to speak effectively before groups of customers or employees of organization. Ability to effectively present information and respond to questions from groups of managers, clients, customers and the general public.

MINIMUM QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE: **

  • Graduate from an accredited school of nursing or graduate program in a related health science field.
  • Two (2) year experience in case management, disease management or managed care preferred. Other comparable experience considered.
  • Knowledge of health care systems and community resources.RN â€" Three to Five (3 â€" 5) years clinical experience in an acute care facility; ICU and/or BH, and emergency care experience preferred.
  • Knowledge of health care systems and community resources desirable.

MATHEMATICAL SKILLS:

    • Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume.
  • Ability to apply concepts of basic algebra and geometry.
  • Mathematical skills with proficiency in analyzing reports and data. * REASONING ABILITY: **
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to interpret an extensive variety of technical instructions in mathematical o r diagram form and deal with several abstract and concrete variables.

LICENSES, CERTIFICATIONS, REGISTRATIONS: **

  • A current unrestricted RN license in the State of Hawai‘i.
  • Current/valid Hawai‘i Driver’s License.
  • Proof of Hawai‘i No-Fault Auto Insurance.
  • Clean driving record as evidenced by a recent driver’s abstract (to be provided by the employee).

OTHER SKILLS AND ABILITIES:

  • Proficient in Microsoft Office including Outlook, Word, and Excel, preferred.
  • Ability to learn to use proprietary in-house systems and databases.

Required education:

  • Bachelor's

Required experience:

  • Clinical in an acute care facility: 3 years
  • Case Management: 2 years
  • Disease Management: 2 years
  • Managed care: 2 years
  • ICU and/or Behavioral Health, and ER care: 3 years

Required language:

  • English

Required license or certification:

  • Registered Nurse (RN)
