The Pharmacist will support the Department of Pharmacy within the inpatient and/or outpatient arena at Tripler Army Medical Center.

The ability to monitor the outpatient dispensing of all pharmaceuticals at Tripler Army Medical Center, Honolulu, HI.
Pharmacists shall perform essentially the same duties as those required of any Government Civil Service Pharmacist of similar experience and in similar duty assignments.Pharmacists shall provide services including, but not limited to the following:
The contract pharmacist will be responsible for development and participation in ambulatory care pharmacy pra! ctice sites and dispensing in the Outpatient Pharmacy. This sh! all be accomplished through the development of collaborative partnerships with the medical staff and reviewing and monitoring outpatient medication regimens.
Daily interactions with prescribers, pharmacist interventions, counter detailing, formulary review, target drug programs, drug utilization and cost effectiveness studies (DUE/DUR), patient education, prevention of adverse outcomes, monitoring for poly-pharmacy patients, monitoring for controlled substance overuse/misuse, etc.
Participate in or conduct studies involving pharmacy related issues in the management of a large outpatient population. Design and conduct studies intended to evaluate particular drugs, their effectiveness and suitability, or to study the reaction of diseases, patients, and conditions to drug-based treatment.
Provide clinical pharmacy services to selected clinics, including drug therapy management, patient education, therapeutic drug monitoring, staff education, nursing in-services and! solicited drug therapy consults.
Provide quality direct patient care by using a pharmacy practice methodology.
Serve as the MTF expert on pharmaceuticals, pharmacokinetics, and pharmacotherapeutics. Uses an extensive and intensive knowledge of the drugs and other pharmaceuticals available, their nature, action, and interaction, provides authoritative instructions, advice, and assistance to prescribing and dispensing officials on effectiveness, suitability, hazards, and other aspects of their use.
Develop, with the attending physician, pharmacotherapeutic regimens for especially difficult or complex cases. Of particular concern are cases involving multiple serious or life threatening conditions with conflicting drug requirements, serious drug reactions or interactions, or other factors not receptive to standard or more common specialized approaches. Monitors progress and response to the regimen, modifying dosages or methods of administering as necessary. In emer! gency situations may change medications or significantly alter regimen ! without
prior approval of physician, but subject to post action review.
Prepare and conduct or oversee training of physicians, pharmacists, and other health care providers on the use of drugs and other pharmaceuticals. Monitor training of residents, interns, and fellows to insure drug related
issues are fully and correctly dealt with in their curriculum and that the required levels of skill have been attained. Recommend or initiate changes and updates to instructions, regulations and other guidance as needed.
Fill prescriptions, including controlled drug prescriptions, by selecting the correct drug and dosage form,reducing the drug from bulk to the quantity prescribed, compounding when necessary, and packaging the drug in appropriate container.
Exercise care in the dispensing of drugs, especially controlled substances, to assure that legal and agency regulations and instructions are followed, that proper and complete record keeping is maintained, and tha! t the drug storage areas are stocked, safe, and secure. Notify the prescriber, who may be on staff or a community physician,
when there are questionable dosages or possible therapeutic incompatibilities based on a review of the patient’s medication profile. Consulting with and advising nurses and members of the medical staff on matters such as
contraindications, side effects, and antagonistic reactions; and suggest alternative medications as appropriate.
Determine the validity of drugs prescribed by the numerous physician extenders, ensuring the drug prescribed is on the particular physician extender’s list.Furnish guidance to prescribing physicians on current Pharmacy and Therapeutics Committee policies, applicable federal and state laws, Army formulas, new preparations, drugs of choice, methods of compounding nonstandard preparations and special formulas suggested or requested by the professional medical staff. Check the work of pharmacy technicians to ass! ure that all drugs dispensed are properly labeled and packaged, and tha! t directions for use and cautions, when necessary, are complete before the drug is dispensed to the patient. Annotate both prescriptions and prescription labels with initials and/or numbered stamp to show dispensing check has been performed.Dispense prescriptions to patients and counsel them concerning their medications, i.e., uses, side effects,directions, etc. Routinely referring to standard reference texts and journals to keep abreast of current trends, new drugs, and new uses and current drugs. Requesting prescriber, usually by telephone to confirm information shown on prescriptions, e.g., when drug strength is questionable. Interpret and evaluate unit dose orders, detecting potential therapeutic incompatibilities and errors, and ensuring unit dose orders are picked up from nursing units. Review and approve technicians’ work. Enter orders into computerized or manual patient profiles, placing medications into unit dose cassettes, and deliver them to nursing units. Inte! rpret sterile product orders, detecting and resolving therapeutic and pharmaceutical problems with physicians and other medical and nursing staff members. Employ appropriate technical skill and/or oversight in the preparation of sterile products which conforms to the standards established by the United States Pharmacopeia, the American Society of Health-System Pharmacists, and the TAMC Department of Pharmacy. Sterile products will include parenteral medications, parenteral nutrition, compounded ophthalmic preparations.
Ensure immediate supervisor is kept informed, set priorities resolving problems and unusual requests,assigning required task to subordinates, providing on-the-job training and professional guidance to technicians. Provide drug information to professional staff in a timely manner. Authenticating all established recordsas required by Pharmacy SOP’s documenting work units, documenting medication errors and maintaining prescription files to include controll! ed drugs and investigational drugs.
Provide technical guidance to a! nd act as preceptor for pharmacy technicians, pharmacy technician students, pharmacy students, and pharmacy residents when these individuals are under the contract employee’s professional supervision. Document clinical pharmacy services and therapeutic consults within the medical record as directed by institutional requirements. Participate in quality assurance activities (e.g. drug utilization evaluations, measures of clinical pharmacist’s impact on drug therapy, etc).Perform pharmacy in-services, teaching and staff development to enhance clinical services,communication and pharmacist education.
Education
Must have a Doctoral of Pharmacy degree from a college of pharmacy accredited by the
American Council on Pharmaceutical Education (ACPE).
Residency training and/or board certification is preferred
Experience
Must have at least four years of post-graduate full-time equivalent experience as an inpatient or clinical Pharmacist ! within the last six years.Must have an extensive knowledge of the drugs and other pharmaceuticals available, their nature, action
and interaction.

Be currently trained, certified and maintain qualification in Cardiopulmonary Resuscitation (CPR) as evidenced by successful hands on training and certification in Basic Life Support (BLS) for Healthcare Providers through an American Heart Association approved training program. LICENSURE/CERTIFICATION/REGISTRATION:
Must possess a valid, unrestricted license to practice pharmacy in any one of the 50 states in the United States or the District of Columbia.
Physical Requirements
Health and immunization requirements. All contract personnel performing services under this contract shall comply with the health and immunization requirements as stated in the TAMC Health Requirements clause.
Hours of scheduled work may include weekdays or weekends and evening and nighttime shifts. Assigned work schedule may not necessarily be consistent and may rotate among days of the week and various work shifts
