Essential Functions:

  • Coordinates planning, scheduling, and implementation of all RAI activities according to regulatory and accreditation requirements.
  • Develops and tracks Minimum Data Set (MDS) care plan notification schedule for each patient.
  • Inputs data into facility's RAI software program.
  • Transmits RAI data to Hawaii State database and maintains transmission files. Ensures accuracy of data transmitted.
  • Collaborates with interdisciplinary team.
  • Completes/coordinates initial MDS assessments, Resident Assessment Protocols (RAPs), and care plans.
  • Plans and facilitates care planning meetings.
  • Reviews assessment protocols with interdisciplinary team.
  • Obtains information from nursing personnel regarding admissions, discharges and significant cha! nges.
  • Makes changes to assessments and care plans whe! n significant changes occur or as needed.
  • Plans and conducts inservice training on RAI activities for nursing personnel and other members of interdisciplinary team.
  • Communicates interdepartmental problems with possible solutions to supervisor.
  • Maintains and keeps abreast of current knowledge of regulatory requirements of RAI industry standards and Medicare Prospective Payment System requirements.
  • Maintains current knowledge and understanding of infection control and safety principles.
  • Assists with review and revisions of RAI policies and procedures.
  • Participates in Quality Improvement activities related to RAI process.
  • Prepares and submits reports to supervisor as requested.
  • May perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.
  • Provides direct patient c! are on an as needed basis.
  • Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.
  • Incorporates the KP Nursing Vision, Model and Values through out their Nursing Practice.
Qualifications

Basic Qualifications:
  • Two (2) years of RN experience in a skilled care facility including coordinating RAI activities.
  • Graduate of an accredited school of nursing.
  • Valid Hawaii RN License (must meet education requirement(s) for Hawaii State licensure).
  • Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification.
  • Knowledge of long term care laws and regulations.
  • Demonstrated knowledge of and skill in adaptability, change management, decision making, influence, interpersonal relations, oral communication, problem solving, teamwork and written communication.
  • Demonstrated knowledge of and skill in word processing, spread! sheet and database PC applications.
Preferred Qualifications:
  • Bachelor's degree in nursing preferred.
