Background and-or Security Investigation required DUTIES:
The VAPIHCS Registered Respiratory Therapist (RRT) is located in the Specialt! y Care Service. The RRT will provide the clinical care and administrative support for patients with respiratory insufficiencies across the VAPIHCS catchment area. The incumbent is a RRT with full responsibility as Program Coordinator. In addition to direct patient respiratory and related care the RRT plans, organized, coordinates, and evaluates a specific program area. The RRT also evaluates plans, develops and implements new procedures or protocols based on the Mission, Vision and Values of the Medical Center and subsequent needs of the program in conjunction with the Supervisor, Respiratory Care Services. Major duties include but are not limited to:

  • Performs as the VAPIHCS staff respiratory therapist;
  • Maintains clinical competence and knowledge of all aspects of Respiratory Care modalities; makes judgment decisions and recommends appropriate respiratory care to physicians;
  • Contributes ideas, plans, and information to improve services;
  • Assists the team in the development and renewal of policies, pr! ocedures, competency assessments, QM/PI, and other areas as necessary;
  • Involves patient in decisions affecting the care provided;
  • Protects printed and electronic files contacting sensitive date in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statues and policy and VHA policy;
  • Provides local VA oversight and clinical review of the Home Respiratory Program according to VHA Clinical Practice Recommendations and local policies and procedures;
  • Demonstrates knowledge of National Patient Safety Goals and Joint Commission standards on home care, fire and safety procedures and other regulations and clinical practice guidelines;
  • Oversees and performs patient respiratory assessments and assists medical staff in determining patientÂ's need of supplemental oxygen and use of conserving devices; coordinates with ordering services such devices used for home sleep studies, and sleep ! apnea treatment;
  • Provides or arranges in-service education for physicians, nurses and other staff as requested or identified as needed.
This position will require travel across the VAPIHCS catchment area, both to Community Based Outpatient Clinics (CBOC) and patientsÂ' homes when necessary, to provide clinical respiratory care services and respiratory care oversight, including on Oahu, the Big Island of Hawaii (Hilo and Kona), Maui, Molokai, Lanai, Kauai, American Samoa, Guam and the Commonwealth of the Northern Mariana Islands (CNMI). QUALIFICATIONS REQUIRED:

BASIC REQUIREMENTS:
a. Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)

b. Registered Respiratory Therapist. Has a certificate as an RRT from the NBRC (National Board for Respiratory Care) and a registry number, or a certificat! e from another body which the NBRC recognizes as its credentialing equi! valent. This includes registration based on either:

(1) Having fulfilled the requirements and passed the registry examination administered by the NBRC since 1983, or

(2) Having fulfilled the requirements and passed the registry examination for a registered inhalation or respiratory therapist administered by the (1) National Board for Respiratory Therapy from July 1974 through 1982 or (2) American Registry of Inhalation Therapists from 1961 through June 1974.

c. Physical Standards. See VA Directive and Handbook 5019 .

d. English Language Proficiency. RRTs appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

GRADE DETERMINATIONS:
In addition to meeting the basic requirements stated above, the following qualificati! ons criteria must be met in determining the appropriate grade assignment of candidates.

a. GS-7. None beyond the basic requirements.

b. GS-8. Candidates must have all of the following:
(1) At least 1 year of successful experience related to respiratory therapy at the GS-7 level or equivalent. Qualifying experience at this level is work which includes duties such as:

  • administering assisted and controlled ventilation to patients with tracheotomies and other complex medical problems requiring frequent adjustments in ventilator parameters;
  • administering and monitoring advanced ventilator techniques such as positive end expiratory pressure and continuous positive airway pressure;
  • developing plans for weaning patients from ventilators;
  • assessing the respiratory status of patients using data acquired through physical observation and clinical analysis of blood gas data, chest x-rays, and electrocardiogram to ! determine the effectiveness of therapy being administered and to make r! ecommendations to physicians regarding changes in treatment;
  • performing the most difficult respiratory therapy procedures to maintain ventilation, including endotracheal intubation, tracheal lavage, and tracheotomy care;
  • and providing in-service training to others in various specialized areas of respiratory care.
(2) Demonstrated knowledge and ability needed to perform complex respiratory procedures with minimal supervision. Is able to participate with physicians and nurses and other staff in planning respiratory treatment, with great reliance on the individualÂ's knowledge of the equipment.

(3) Demonstrated knowledge and ability to provide emergency or critical respiratory care; service as a shift leader or supervisor or sole responsible respiratory therapist on a shift; and to plan and conduct training sessions with respiratory therapy students, hospital staff, patients, and family members.

Physical Requirements:
  • The work requires st! anding and waling for prolonged periods of time. The respiratory therapist must frequently bend, stoop, run, reach and stretch to set-up and disassemble respiratory therapy equipment, position and move patients, respond to emergencies and push/pull light to moderately heavy objects such as mechanical ventilators;
  • Must have visual acuity and hand eye coordination necessary to perform intricate clinical tasks in confined areas and ability to perform auscultation to access breath and cardiac sounds.
