Under indirect supervision, assumes responsibility to evaluate, develop, facilitate, and implement individualized therapeutic exercise and treatment plan in patient's h! ome. Collaborates with health care team, patient, and caregivers to assure occupational therapy regime is safe, agreeable, and appropriate to patient. Provides direct care and supervises Physical Therapy Aides (PTAs) and Home Care Aides (HCA) involved in plan of care. Maintains professional, clinical, technical, and interpersonal skills to provide best possible service.

Essential Functions:

  • Evaluates patients, families, and home environment on admission to home health and subsequent home visits; determines if home environment is safe and appropriate for provision of home health services; demonstrates ability to interpret data about patient's status: assesses patient's physical, functional and psychosocial status; identifies problems accurately; establishes reasonable/measurable goals; develops and/or revises an appropriate plan of care; communicates and coordinates plan of care with physician, patient, caregivers, and other health care providers! ; implements care, as ordered; determines whether other servic! es such as nursing, OT, ST, HCA or MSW are indicated; establishes discharge planning in conjunction with the care planning process; prepares and delivers medical supplies and equipment necessary for provision of prescribed treatment
  • Provides occupational therapy/exercises as prescribed by patient's physician and in compliance with Kaiser Permanente Home Health Agency (KPHHA) standards, policies and procedures. Evaluates patient response to interventions
  • Develops home therapy program for each patient that provides for continuation of treatment in absence of physical therapist/PTA
  • Recognizes and interprets significant physiological and behavioral changes in patient's condition. demonstrates critical thinking such as problem solving, reflective practice, clinical reasoning, prioritizing, etc.
  • Determines in conjunction with physician, when patient care needs to be shifted to another setting, e.g., hospital, clinic, emergency department, out-p! atient rehab, etc., and/or patient denial of home health, if warranted
  • Provides all care according to established, written plan of care: modifies/updates/changes plan of care when indicated by patient's condition and/or physician's orders; obtains physician orders, as required, by State and Federal laws
  • Provides case management, as caseload assigned; coordinates all necessary services to be provided to patients; integrates all services into plan of care
  • Provides guidance and supervision to PTA; reevaluates patient's therapy needs regularly when PTA furnishes services
  • Supervises care provided by Home Care Aides (HCA); prepares HCA assignments; coordinates patient care, makes supervisory visits for HCA services and provides feedback to HCA; documents HCA supervision

Qualifications

Basic Qualifications:
  • Two years of recen! t physical therapy experience
  • Bachelor's degree in Physical Th! erapy
  • Valid Hawaii Physical Therapist license (must meet education requirement(s) for Hawaii State licensure)
  • Current BLS for Healthcare Provider CPR or CPR/AED for the Professional Rescuer certification
  • National Provider Identifier (NPI) and Taxonomy code required at time of hire• Valid driver's license
  • Ability to physically access variety of terrain and assist patients/caregivers in performing functional tasks and therapeutic exercises
  • Demonstrated knowledge of and skill in interpersonal relations, oral communication, written communication, customer service, and problem solving
  • Demonstrated knowledge of and skill in word processing, spreadsheet, and database PC applications
  • Talking to co-workers, customers, outside vendors, on the telephone, and supervising others
  • Reading, writing, speaking, and understanding English
  • Training/giving and receiving instructions
  • Mathematical ability, ! attention to detail (e.g., organization, prioritization, proofing), concentration, and alertness
Preferred Qualifications:
  • Three years of home health care experience
  • Master's degree in Physical Therapy
